i think this is the time you need to be patient, you already try to do "clean up" as much as you can... but IMO, there are some aspect that happening right now:
1. GoogleBot itself is re-crawling and calculate your site day by day.
2. Disavow Tools is not an instant solution
3. People believe there are two type of penalty: Manual and Algo Pinalty.. perhaps your site has been flagged by algo pinalty..

There are many reasons to decrease the ranking. Some of the important points to refer
1. Competitors gets quality backlinks
2. Loss or expire the quality backlinks for your website
3. Getting low quality or bad backlinks for the website etc....
