The SmartFormer Gold extension for Magento is a powerful tool that will amplify the default Magento functionality and take your custom forms to a new level. The form builder is developed for any level of users. The extension includes draggable HTML elements, toolbox of snippets and custom email templates, integrated PHP, CSS, JS and HTML editors for advanced use to change or customize the layout or behavior of forms. The Database Manager will help you collect and store customer information.
There are two more extensions that work in compatibility with the SmartFormer Gold extension:

1. Digital signature for Magento forms adds the digital signature functionality to your forms. You are not required with special tools. After the signature is added, it would be converted into image. Signatures can be saved in the Database as well.

2. Advanced PDF plugin for Magento forms extension is table-based export of your forms’ submissions to PDF. Submissions can be exported to the PDF files. You will be able to create a PDF outlook, insert images, texts, or submissions’ data to PDF.
